He's fast. Far faster than Garak could have anticipated and while he's a little off-balance Garak's mostly just relieved to not get burned again. Or at least not literally burned. The gun pointing at him threatens to be quite the metaphorical burn. He should have simply struck. He knew Julian was going to be breaking the law sooner or later, it was his duty to stop the doctor, and he's failed.
His only consolation is the fact that he still has a chance, because he knows Julian won't shoot him. He's far too gentle for that.
